This form is for Poets and Songpoem writers wishing to post poems and lyrics for sale, licensing or as samples of your work.

Instructions:

• Complete and print out this form.
• Enclose typed poem(s) (up to 2 pages ). Please include title and genre of each poem.
  You may also submit poems on disk or by e-mail. E-mailed poems will be posted to our site as soon as check is received.
• Enclose $29.00 check or money order for first two typed pages and $10.00 for each additional page for a one-year listing.
